I wouldn’t necessarily say poor leadership, however there are issues that obviously need to be addressed. However, this case is over 15 years old and this was a common problem among SWAT operations. If you think that this was the only barricade that entry was made then you are sadly mistaken. Our tactics needed changed then and unfortunately still need addressed. You can’t make entry into someone’s house and kill them to save them which is the bottom line here. Most teams have changed and this call wouldn’t even amount to a SWAT call out. However 15+ years ago this was normal in many areas of our country. This is a failure to train for officers in dealing with mental illness, which is a department problem that is still being addressed today as mental illness is a major problem yet.
